Enrollment: Prairie Five Head Start is
funded for 231 Federal children and 32 State children. The total
number of children and families served was 278, In the 2009-2010
school year 23% of the total enrollment was children with
disabilities. Prairie Five Head Start serves children in five
counties. There are 24 classes in 14 communities and 2 child care
center partners. The average monthly enrollment for 2009-2010 was
90%. Over 90% of of eligible children in the five county area were
served. We had a waiting list of 30.

Monitoring: Prairie Five Head Start completes an annual self
assessment. Prairie Five is also monitored every 3 years by the Federal
government, the State Department of Education, and the State Food and
Nutrition Service. During November 2008 the Federal Review Team monitored
the program. The program was in full compliance with all Federal
requirements.
State Food and Nutrition Service monitored our program in May 2009. No
major problems were found in the annual audit. Audit information

Health Services: In the 2009-2010 school year, 100% of
enrolled children had up-to-date immunizations; 100% of enrolled children
had completed physicals; 99% of enrolled children completed dental
examinations.
Parent Involvement: During the 2009-2010 school year Prairie
Five Head Start had 513 parent and community volunteers who volunteered
a total of 10,593 hours for an in-kind value of $118,237. 155 parents
and community members participated in Parent Committee and Policy
Council meetings a total of 1019 hours for an in-kind value of $24,446.
Prairie Five Head Start has an active Policy Council made up of parents
and community members. Highlights of parent activities are the
annual "Rewards Trip" and the annual trip to the State Capitol for
children's advocacy day.
Child Assessment & Outcomes: Prairie Five Head Start uses the
Creative Curriculum and assessment tools. Assessments are completed 3
times during the year and outcomes are shared with parents. Our
curriculum and assessment prepares children for Kindergarten. Program
