MNParentsKnow.infoThe Minnesota Department of Education (MDE) announces the MNParentsKnow.info Web site has been expanded to include research-based information for parents of children from birth through age 5.
The expansion of Minnesota Parents Know supports Minnesota’s continued commitment to supporting parents as their child's first and most important teacher.
The Minnesota Parents Know Web site, which offers Minnesota parents a convenient source for trusted knowledge on their child’s health, learning, development and safety. The information added for ages 1 through 5 is in addition to the infant information already available on the Web site. Ultimately, the department plans to include information through grade 12.
MNParentsKnow.info is hosted by the MDE and was developed as part of the Educate Parents Partnership, an early childhood initiative of Gov. Tim Pawlenty funded by the 2006 Legislature. The Web site was developed by the Department of Education in collaboration with the Department of Health, Department of Human Services and many other public and private organizations.

This family and child development program serves low-income families and children with disabilities with family assessment, goal setting, medical, dental, mental health, and nutrition services. The programs are combination which means the children come to the center 2 to 3 days a week and have 1 to 2 home visits each month. Family partnerships and goals are developed for children and families.

Outreach Workers/Clothing Centers/Food Shelves:
Prairie Five Community Action Council, Inc. employs a full time Outreach Worker in each of the five counties the agency serves. These Outreach Workers are trained and are knowledgeable in all services provided by Prairie Five. They also are able to make referrals to other service providers in the area. Depending on current funding the Outreach Workers assist customers in applying for heat and utility shut-offs, threatened eviction, emergency food and shelter, other crisis situations and provide help in filing taxes and health insurance counseling.

Child Care Resource & Referral is a community based organization, delivering person-to person services that include comprehensive information, child care referrals, and training growth, as well as supporting parents in their search for quality care by developing resources that are responsive to our community needs. Our primary goal is to ensure that all children have access to quality, nurturing environments. Equipment and book/video lending library is available to all providers. Weatherization Program
The Weatherization Program reduces the impact of rising energy costs through the installation of energy conserving measures. Also, health and safety issues are addressed by checking fuel burning appliances for carbon monoxide emissions and checking indoor air quality.
The Family Development Program
The Family Development Program strives to assist individuals to become self-supporting. The main emphasis is presently advocacy/listening and referrals.
DEED (Department of Employment and Economic Development)
These are income based deferred grants requiring matching funds from the homeowner for basic necessary housing repair. Funds are distributed on a first come first serve basis and receive matching funds. Contact the housing program to find out where current grants are being administered.
Closing Costs Assistance Program
Serving the City of Montevideo with closing costs assistance through a deferred loan program. Also referrals are made to the Southwest Minnesota Housing Partnership who administers other Affordable Housing Loan Programs in Prairie Five’s service area.
FAIM Program
(Family Assets for Independence in Minnesota)
The FAIM Program, also known as Individual Development Accounts (IDAs), helps low-wage earners build assets through home purchase, secondary education (for the applicant or an eligible child) or starting a small business. Account holders save funds, up to $480 per year, which is matched at a 3:1 rate for two years. Account holders must complete financial management classes and specific asset building classes in order to receive matching funds.

Energy Assistance Program
Energy Assistance Program is a federally and state funded program that assists low-income households in meeting their home heating costs and to promote self-sufficiency through low cost energy conservation activities and consumer education. The program is designed to provide for the equitable distribution of assistance without eliminating household responsibility for paying energy bills. EAP funds are made available to eligible households through local delivery agencies, to pay a portion of their costs for home energy, especially during the heating season. Funds are also available for weatherization activities and energy-related repairs. Energy Assistance payments are calculated to provide the highest level of benefits to household with the lowest incomes and the highest energy costs based on fuel types.
Crisis Repair Program
Crisis Repair Program is a component of the Energy Assistance Program. The Crisis Repair Program helps households that are experiencing problems or are in a no-heat situation to repair or replace the heating source. Each case is handled individually as the clients have different needs that need to be addressed.
Crisis Non-Repair Program
Crisis Non-Repair Program helps households that have no resources available to help resolve a heating or an electrical emergency during the winter months. This may involve paying an electrical disconnect, natural gas disconnect, or ordering and paying for additional fuel or oil or propane. The Crisis Non-Repair Program works with the vendor to resolve problems and will make referrals to other service providers when necessary.
Reach Out For Warmth
Reach Out For Warmth provides people in need with emergency assistance and emergency furnace repair. It is a community-based fuel fund with its success dependent upon the growing support of individuals, businesses, churches, civic groups, school children, energy vendors, and private foundations. Tax deductible donations are matched two dollars-to-one dollar.

The Prairie Five RIDES Transit System is a demand responsive combination of volunteer driver and bus service providing public transportation with the counties of Big Stone, Chippewa, Lac qui Parle, Swift and Yellow Medicine. Points of destination outside the five counties with the buses are Minneapolis, Willmar, St. Cloud, Marshall. Rides can be provided by volunteer drivers to destinations buses do not go. Prairie Five RIDES operates city transit systems in Appleton, Canby, Madison and Ortonville.

Senior Nutrition-More Than A Meal...
This program promotes health and independent living in the senior population through improved nutrition and reduction of social isolation. There is an established suggested donation for meals, but program participants may pay what they can afford. Congregate Meals offers nutritionally balanced meals at senior dining sites throughout Region 6W. Health information, referrals, nutrition education, volunteer opportunities and companionship with other seniors are also available at the dining sites. Home Delivered Meals is a vital service, enabling homebound seniors to live healthier and independently in their own homes. Home delivered meals are available on a short/and/or long term basis. Frozen meals are also available for weekend, holiday or rural use. Local volunteers deliver meals to a customer’s home. The Senior Nutrition Program offers a warm and caring atmosphere with friendship and fun. There are volunteer opportunities awaiting those who would like to be actively involved.

Education For Families In Divorce Transition
Parents Forever offers education and resources for married families going through divorce and never-been-married families going through a separation. Divorce and separation often brings change to every aspect of a family's life, both for the parents and the children involved.
Parents Forever provides parents with formal training programs and easy-to-use
materials that help them navigate the emotional, financial, legal, and communication changes that accompany divorce/separation so that they can effectively help their family transition. Many of these materials have been culturally adapted for Spanish-speaking families. The program also trains family professionals to be better equipped to help during the transition.

Small World Nursery School is a Community Education program for children three to five years of age who are not yet enrolled in Kindergarten. The nursery school program is designed to assist parents with the emotional, social, intellectual and physical development of their preschool child. Parents Forever offers education and resources for married families going through divorce and never-been-married families going through a separation. Divorce and separation often brings change to every aspect of a family's life, both for the parents and the children involved.
Parents Forever provides parents with formal training programs and easy-to-use materials that help them navigate the emotional, financial, legal, and communication changes that accompany divorce/separation so that they can effectively help their family transition. Parents Forever offers education and resources for married families going through divorce and never-been-married families going through a separation. Divorce and separation often brings change to every aspect of a family's life, both for the parents and the children involved.
Parents Forever provides parents with formal training programs and easy-to-use materials that help them navigate the emotional, financial, legal, and communication changes that accompany divorce/separation so that they can effectively help their family transition. Many of these materials have been culturally adapted for Spanish-speaking families. The program also trains family professionals to be better equipped to help during the transition.

Parents Forever™ offers education and resources for married families going through divorce and never-been-married families going through a separation. Divorce and separation often brings change to every aspect of a family’s life, both for the parents and the children involved.
Parents Forever™ provides parents with formal training programs and easy-to-use materials that help them navigate the emotional, financial, legal, and communication changes that accompany divorce/separation so that they can effectively help their family transition. Many of these materials have been culturally adapted for Spanish-speaking families. The program also trains family professionals to be better equipped to help during the transition.

IEIC’s are a part of the system for providing special services to children age 0-21. This special early care is sometimes called early intervention. IEIC members are parents and professionals from the community. IEIC’s look at concerns of families who have children with special needs. IEIC’s have tasks set down by Minnesota law, which include knowing the services available in their community. IEIC members may also know parents who share your experience. IEIC’s need parents to be involved in the committees to make sure that children’s and families needs are being met. PACER Center, Inc., advocates for parents of children with any kind of disability, ages birth to 21 years. PACER also instructs families on becoming their child's advocate. PACER teaches parents their legal rights in seeking special education services and provides information about education rights. The Health Information and Advocacy Center project provides a central source for families of children and young adults with disabilities to obtain support, advocacy and information about the health care system. Some of the information provided includes understanding insurance and filing insurance appeals, communicating with health care providers, making informed choices about health care providers and general health resources. PACER offers workshops for parents and professionals, links with parent-to-parent support programs, puppet shows on disability sensitivity and preventing child abuse. Having someone step in and care for your child can stop the strain from getting the best of you. One kind of short-term care is called respite care. There are many ways of getting respite care. Education For Families In Divorce Transition
Parents Forever offers education and resources for married families going through divorce and never-been-married families going through a separation. Divorce and separation often brings change to every aspect of a family's life, both for the parents and the children involved.
Parents Forever provides parents with formal training programs and easy-to-use materials that help them navigate the emotional, financial, legal, and communication changes that accompany divorce/separation so that they can effectively help their family transition. Many of these materials have been culturally adapted for Spanish-speaking families. The program also trains family professionals to be better equipped to help during the transition.
SUPPORT / NETWORK GROUPS
Support groups are meetings for people who share the same concern. In the case of parents who have children with special medical needs, the concern might focus on the child's specific disability or the needs of the child. Groups vary not only in concern, but also in their length of time. Some groups meet only once. Some meet for just a short number of weeks. Others go on for years. Because they come and go, it is difficult to list groups for an area. If you are interested in finding a support group, you may have to call around.

Waiver programs allow family income requirements to be "waived" (not counted) in order to receive MA services. Home and community-based waiver programs are designed to serve individuals with special needs in the community who would otherwise require the level of care provided in a nursing home or other health care facility. There are requirements the individual must meet to be eligible for a waiver program. Covered services under the home and community-based waiver programs include all covered MA services plus home health care, therapies, equipment, supportive employment, foster care, homemaking, home/vehicle modifications, respite, counseling, and others.
TAX EQUITY AND FISCAL RESPONSIBILITY ACT (TEFRA)
All individuals applying for MA must meet certain conditions to become eligible. MA is available for some disabled children who usually would not be eligible because of parent's income. This is the Tax Equity and Fiscal Responsibility Act or the TEFRA option. Children are eligible with regard to their own assets but not assets of their parents. Some parents may be required to pay a fee (co-pay) based on their income.
For a child to be eligible for MA under the TEFRA option the child must:
∙ Be under 18 years of age
∙ Be certified disabled by the State Medical Review Team (SMRT)
∙ Be eligible for MA as if they were living in a medical institution
∙ Meet all other MA eligibility factors

MINNESOTA CHILDREN WITH SPECIAL HEALTH NEEDS (MCSHN)
Minnesota Children with Special Health Needs (MCSHN) is part of the Minnesota Department of Health. MCSHN helps Minnesota families with children under age 21 who have special health needs or disabilities. MCSHN has different types of programs. MCSHN can pay for diagnostic services. These are tests or examinations to see if a child has a medical problem and what would be done about it. There is no family income requirements for evaluations, but insurance must be used first if available. Evaluations must be approved ahead of time so it is important to consult MCSHN early. MCSHN can also fund treatment of special medical care if a child has a special health need. To receive MCSHN treatment services, a family must meet MCSHN's income guidelines. The child must have a medical condition requiring on-going care. MCSHN can begin to pay for medical care the date of the initial referral provided the completed application is returned within 30 days of being sent to the family. MCSHN can be a helpful resource. Their consultants have experience and knowledge about services in the community. They have information about many medical conditions.
The Information and Referral Line can help families, health care providers, and community programs find the right resource to meet special needs. The service is free and confidential. It is available week days from 8:00-4:40 p.m.

PACER CENTER, Inc.
PACER Center, Inc., advocates for parents of children with any kind of disability, ages birth to 21 years. PACER also instructs families on becoming their child's advocate. PACER teaches parents their legal rights in seeking special education services and provides information about education rights. The Health Information and Advocacy Center project provides a central source for families of children and young adults with disabilities to obtain support, advocacy and information about the health care system. Some of the information provided includes understanding insurance and filing insurance appeals, communicating with health care providers, making informed choices about health care providers and general health resources. Taking care of any child can be a tiring and stressful job at times. For parents of a child who has special care needs, the job can be more difficult and the chances to relax are much fewer. MCHA was established in 1976 by the Minnesota Legislature to offer individual health insurance policies to Minnesota residents who have been turned down for health insurance by the private market due to pre-existing health conditions. SUPPLEMENTAL SECURITY INCOME (SSI)
SSI is a program through Social Security for people with disabilities or special health needs. SSI provides a monthly payment to use in meeting that child's needs. The individual must qualify medically and financially. SSI requires the family to have a low income and few assets.
